How do I get the scratches out of my discs?

I have CD's, DVDs, and games galore, and I take excellent care of them. However, my PS2 has messed a lot of them up, not only with lots of small scratches, but also a highly resistant ring around the outside edge that I can't get out even the slightest, which I call the Clear Ring of Death. I can get the small scratches out, no problem, but they're back in 5 minutes. I've tried a lot of stuff, but nothing will work. Any suggestions?
